Duty of Candour Statement

P4C Healthcare Ltd is committed to providing safe, high-quality care and to being open, honest and transparent when things go wrong. In accordance with the Health (Tobacco, Nicotine etc. and Care) (Scotland) Act 2016 and the Duty of Candour Procedure (Scotland) Regulations 2018, we have prepared and published our annual Duty of Candour Report for the reporting period. The report is available on request from our office and has been made available alongside this statement.

P4C Healthcare Ltd is committed to ensuring that all people receiving our services are treated with honesty, respect and compassion. We recognise the importance of being open and transparent whenever an unintended or unexpected incident results in, or could result in, harm.

Although there were no incidents requiring the formal Duty of Candour procedure during this reporting period, P4C Healthcare Ltd continues to:

  • Promote a culture of openness, honesty and learning.
  • Encourage staff to report incidents, concerns and near misses.
  • Investigate incidents where appropriate.
  • Provide Duty of Candour and incident reporting training to staff.
  • Review policies and procedures regularly to support continuous improvement.

This report has been approved by the management of P4C Healthcare Ltd and is published in accordance with the Duty of Candour Procedure (Scotland) Regulations 2018.
